INDIVIDUAL ASSESSMENT NARRATIVE FORM

Each year all full-time faculty and appropriate staff are required to complete an individual assessment narrative describing their assessment practices for that year. The assessment narrative may describe a project used to enter data into the Campus Assessment Portal (CAP) or may describe other assessment projects or activities.

DEPARTMENT/PROGRAM/CO-CURRICULAR ANNUAL ASSESSMENT REPORT FORM

Each year department chairs, program directors, and co-curricular area heads are required to complete the Annual Assessment Report Form. If you are in charge of several disciplines or programs you may fill out this form for each separately, or you may use one form and include information for multiple programs/disciplines.
