Complaint/Non-Instructional Grievance

This form allows you to address a non-instructional concern/complaint related to Student Affairs or Administrative Services departments. Please be specific in documenting your concerns; they will be treated discretely and confidentially.

Complaint/Instructional Grievance

This form allows you to address a concern/complaint related to an Instructional Grievance at STEP 3 (VPAA/Dean of Academic Affairs). Please ensure STEP 1 (discussion with faculty) and STEP 2 (written complaint to Faculty/Department Chair) have been followed prior to completing this report.
