Request an Appointment ( Instructor Exam)

I understand I must allow at least one full business day (24 hours) to process my appointment request.

I understand I must check in at least 5 minutes prior to my appointment; students who arrive late may be required to reschedule their appointment.

I understand I must present a current, valid photo ID (state, federal or college ID) and a printed/digital copy of my appointment confirmation email when I check in for my test.

I understand the last name on my photo ID and the name in my Student Center (my.maricopa.edu) must match exactly. If they don't match I must get it corrected in Admissions prior to checking in with Testing Services.

I understand that all of my personal belongings (phone, watch, backpack, etc.) must be stored in the Testing Services lockers while I am completing my exam. Electronic devices (cell phone, computer, etc.) must be turned off and stored during my exam.

I understand that children are not allowed in the testing room, nor may children (under the age of 18) be left unattended while I complete my exam

I understand that it is my responsibility to know if my instructor allows resources (note cards, calculators, etc.) on my exam, if I have any questions I will contact my instructor prior to my appointment.

I understand all testing is subject to video and audio recording and surveillance. By agreeing to receive services from Testing Services, testing clients are acknowledging their compliance and agreement to the policies and procedures of Testing Services.
