This is a list of financial aid forms by aid year. For a current list of items being requested from you, please check for outstanding To Do List items in your Student Center Tasks tile. Make sure each form contains all of the required information and signatures, submit any other required documentation as requested. Please make sure to include your Student ID on all forms and documents submitted.

- Up to 14 business days (**Processing times may take longer during peak)
- DEADLINE: For loan changes, this form must be received no later than 7 business days before your latest class end date. If you are no longer eligible for a loan, the request will not be processed.

2026-2027 Financial Aid Forms
Financial Aid forms for the 2026-2027 academic year are now available through GTeForms, our new electronic forms system, in your Student Center.
To access your required forms:
- Log into the Student Center using your MEID and password, and check the dropdown at the upper left to ensure you are on the Student Center Homepage.
- Click on My Student Forms
- Click on Financial Aid Forms
- On the left-hand menu, you will be able to view and complete any required financial aid forms directly through GTeForms.
Students wanting to request a federal student loan must submit a 2026-2027 Financial Aid Revision Request form, and select the "Request Initial Loan" from the dropdown menu.
Once we receive your request, the Financial Aid Office will review your loan eligibility, determine the amount you may qualify to borrow, and update your financial aid offer. You will then see the loan in your Student Center.
You will then need to:
- Accept the loan amounts in your Student Center.
- Monitor your To Do List for additional requirements, such as Master Promissory Note and/or Entrance Counseling.
Please keep in mind that submitting a request does not guarantee you will receive a student loan. Your eligibility and loan amount are determined by federal requirements, including your enrollment level, cost of attendance, annual and aggregate loan limits, and other eligibility factors.
