Academic Probation Form

If you have received a notification of Academic Probation in your Message Center, your Enrollment for the next semester is limited to 12 credit hours. If you wish to enroll or remain enrolled in more than 12 credits, you will need to submit an appeal form (12 credit) to the Admissions, Records & Registration.

If you have received a notification of Academic Continued Probation in your Message Center, your enrollment for the next semester is limited to 6 credit hours. If you wish to enroll or remain enrolled in more than 6 credits, you will need to submit an appeal form (6 credit) to the Admissions, Records & Registration.

Because your success at Phoenix College is important to us, you should be aware of the importance of improving your cumulative grade point average. Failure to raise your grade point average above the minimum scholastic standard will result in continuation of Academic Probation and may result in continued limitations for future enrollments.

We strongly encourage you to speak to an Academic Advisor about what classes would be the best ones for you to take or retake to get you back in good academic standing. You may also benefit from talking with a Counselor about your career and educational goals or any personal issues that may be preventing you from being successful.
